Presenting Organization: Livewell Kershaw Coalition
Submission Title: All Patients with Diabetes are Empowered to Improve Health Outcomes
Contest Result: 2021 Finalist

Submission Overview:  The Livewell Kershaw Coalition works to foster collaboration between citizens of all ages, working in partnership with public and private sector organizations and schools to increase community awareness of health issues. Their Turn the Curve submission found that rates of diabetes in Kershaw County, South Carolina were not equally distributed among racial and ethnic groups. Particularly, the coalition noted that diabetes was most prevalent among Hispanic/Latinx populations, and that it was rising steadily. Food insecurity, affordability of medication, lack of transportation, and language barriers were found to be among the most notable limiting factors for the population. With these factors in mind, the Livewell Kershaw Coalition set out to create wider support systems, and to partner with community members to increase public health in Latinx communities. They partnered with food share programs and public transportation efforts were partnered with, and the coalition created community health clinics to ensure that the targeted communities were involved in creating their own solution.
