Why use a documentation tool?

After applying for accreditation or reaccreditation, your health department will have a limited time to submit the required documentation.

Documentation is an extensive, on-going process (Reaccreditation alone requires the submission of 90+ documents!)

Organize Documents by Domain and Measure

Instead of searching through endless PDFs and word documents on your computer, easily access documents grouped by PHAB Domain and Measure.

We’ve already grouped all the requirements and measures together by domain, so you’re halfway there! All you have to do is create, edit, and save each document in the appropriate place.

Create and Edit Your Documents Right in the Software

Using our text editor, you can create, edit, and save your documents right on each measure page.

Customization is endless –

  • create tables and bulleted lists,
  • insert pictures and links,
  • change font type, size, and color
  • edit using the HTML editor

Accidentally deleted a portion of your document you weren’t supposed to? No problem! We automatically save your documents in an archive so that you can go back in time to restore earlier versions.

For each reaccreditation document required you will find:

  1. The type of document required (narrative, example, plan, template, etc.)
  2. If examples are required, the number that are required
  3. The official guidance sourced directly from the PHAB accreditation guides
  4. The timeframe from which documentation is required (eg. less than 5 years ago, etc.)

Make the work of outcomes reporting easier with the ability to quickly create health indicators, organize them into Scorecards, and enter data on the fly.
