PARTNERSHIP AGREEMENT BETWEEN
United Way of the Mid-Willamette Valley (UWMWV) and
Yamhill Community Care Organization Early Learning Hub (YCCOELH)
Introduction: This Partnership Agreement formalizes a mutual goal of United Way of the Mid-Willamette Valley and Yamhill Community Care Organization Early Learning Hub to provide transportation and gas vouchers to individuals who lack needed transportation for medical appointments, Department of Human Services or WIC appointments, or employment interviews, counseling appointment, parenting educational classes, teacher-parents appointments and other social services appointments within Yamhill County and close vicinities. This endeavor will enhance and support the work of non-profit organizations that work directly with this target population.
UWMWV and YCCOELH understand the importance of reliable access to transportation, which in rural communities can be a significant barrier for adults and youth who do not have their own transportation to critical appointments and job interviews. UWMWV will put an emphasis on providing financial support to non-profits in Yamhill County to support families and youth with transportation needs.
Scope of the Partnership: UWMWV and YCCOELH will each allocate $3000.00 for a total of $6000.00 for the 2014 – 2015 fiscal year starting July 1, 2014 to June 31, 2015. The allocated funds will be used exclusively for the purchase of bus passes, gas vouchers, or other resources that will provide access to transportation for medical appointments, Department of Human Services or WIC, counseling appointment, parenting educational classes, teacher-parents appointments and other social services appointments, or employment interviews.
UWMWV will remit a check for $3000.00 to YCCOELH for this specific purpose. YCCOELH will match dollar for dollar the $3000.00, for a total investment of $6000.00 to provide transportation services to individuals (adults and youth) in Yamhill County.
The funds allocated through this Partnership Agreement will build capacity of Yamhill County non-profit organizations to better serve their clients and ensure that in Yamhill County:
Adult and youth are healthy, and making healthy choices,
Adults and youth feel safe and supported in their community,
Adults and youth are better able to obtain good jobs and have more opportunities to succeed,
Adults and youth are responsible for their wellbeing.
Time Frame and Association: This Partnership Agreement will remain in effect until such time that either UWMWV or YCCOELH agree to revise it. The Partnership Agreement will be reviewed by each party annually with the intent to maintain similar funding and continue to provide this resource.
This Partnership Agreement is a nonbinding agreement that the Parties have entered into in good faith. A party may disassociate from the effort without penalty or liability by providing the other party with written notice. Written notice shall be sent thirty (30) days prior to the disassociation.
This Partnership Agreement in no way restricts UWMWV or YCCOELH from participating in similar activities with other public or private agencies, organizations and individuals.
Specific Roles and Responsibilities of Participants:
UWMWV agrees to:
• To provide funding for the project
• Promote this partnership between UWMWV and YCCOELH with other stakeholders in the region.
• Identify additional partners and stakeholders.
• Attend meetings as requested by YCCOELH.
• Engage in a positive public relations and communications with YCCOELH.
• Coordinate services with leaders of partner organizations in Yamhill County.
• Maintain public accountability across all partners to improve the wellbeing of all communities in Yamhill County.
YCCOELH agrees to:
• Match UWMWV allocated amount, dollar for dollar.
• Manage the funds by purchasing bus passes, gas vouchers or other tools to provide transportation as described above.
• Distribute bus passes, gas vouchers or other tools to non-profit agencies that work directly with the public in Yamhill County.
• Provide reports to UWMWV every six months, or as requested by the Community Engagement & Impact Director. Reports shall include number of bus passes, gas voucher or other means of transportation; number of adults and youth served.
• Maintain public accountability across all partners to improve the wellbeing of all community members in Yamhill County.
• Promote this partnership with other stakeholders in the region; identify more partners and stakeholders to invest in this project; engage in a positive public relations and communications with UWMWV.
• Be responsible for their respective data entry and process to improve service delivery and coordination with other agencies in using the transportation vouchers.
